Effect of COAP and VAMP Combination Chemotherapy in Acute Leukemias

Abstract

|
The discovery of combination chemotherapy with ara-C and anthracyclin gave a major
advance in the treatment of adult a cute leukemias, with high complete remission rate
and prolongation of duration of complete remission and their survivals. In Korea patients
with a cute leukemia usually refuse the anticancer chemotherapy because Of its poor
prognosis and their low economic state. Also they are easily lost to be followed up, and
another important factor that causes high mortality is inadequate supportive care. In 26
cases of ANLL with COAP therapy and 15 cases of ALL with VAMP therapy in
National Medical Center from July 1976 to June 1981, the following results could be
obtained.
1. The mean age wag 38 in ANLL and 29.3 in ALL.
2. Twenty-seven percent of ANLL and 60% of ALL achieved complete remission,
whereas adequate trials gave 43.8% of complete remission in ANLL and 90% in ALL.
3. The complete remission rate was 38.5% in patients who were under 35 years of
age, and 15.4 % in patients above 35 years in ANLL. But no significant difference in
ALL.
4. The mean number of cycles to complete remission was 4. 3 in ANLL and 4 in
ALL.
5. Complete remission rate according to the FAB classification in patients with
adequate trial of chemotherapy was 66. 6% in Ml and 50% in
M2.
6. Four patients with ANLL and 3 Patients with ALL are still alive and followed up.
7. Two main causes of death are infection and bleeding.
8. Postmortem study showed the leukemic infiltration of the liver in 3 of the 4
patients
with ANLL. One patient with ANLL and one of the 2 patients with ALL revealed the
leukemic cells in the spinal fluid.
